2nd degree heart block type I












The rhythm is irregular with a rate of 40/min.   The P waves are wide and show some splitting, a characteristic of left atrial enlargement.   There is progressive lengthening of the PR interval.  A nonconducted P wave follows the 2nd and 4th complexes.  The QRS complex is narrow.  PR:  .20/.28/.36 sec.  QRS:   .08 sec,  QT:  .44 sec.  Interpretation:  2nd degree heart block type I

Atrial flutter












The rhythm is regular with a ventricular rate of 44/min.   There are flutter waves which are seen better in the V1 lead.   There are 3:1 flutter waves for every QRS complex.   The atrial rate is 188/min.   The QRS complex is wide.  There is 5 mm ST elevation in lead II.  PR:  ---,  QRS  .12 sec.  QT:  .44 sec.  Interpretation:  Atrial flutter

Normal sinus rhythm













The rhythm is regular.   The heart rate is 93/min.   The P waves are upright and are associated with a QRS complex.   The QRS complexes are narrow.  There is 1mm ST depression with T wave inversion in lead II.  No ectopic beats are noted.  PR:  .16 sec,  QRS:  .08 sec.  QT:  .36 sec.  Interpretation.   Normal sinus rhythm.

Normal sinus rhythm with unifocal PVCs











The rhythm is irregular and the rate is 100/min.   The P waves are associated with a QRS complex and have a uniform morphology.   There are unifocal PVCs every 5th beat.   PR:  .16 sec.  QRS:  .08 sec.  QT:  .36 sec.  Interpretation:  Normal sinus rhythm with unifocal PVCs

Normal sinus rhythm with a multifocal couplet














The rhythm is irregular. The heart rate is 90/min.  The P waves show some splitting but they are uniform and upright.   The 2nd and 3rd complexes represent a multifocal couplet.  The last complex has the same morphology as the first PVC so this suggests that they came from the same source.   PR:  .20 sec.  QRS:  .08 sec.  QT:  .44 sec.  Interpretation:  Normal sinus rhythm with a multifocal couplet
