Practice EKG Rhythm Strips 224

Identify the following rhythms.

1.

2.

3.

4.

5.

Answers

1.
2nd degree AV block type II

The rhythm is irregular with a rate of 40/min.  The P waves are upright and uniform but as you can see not all of them are associated with a QRS complex.  The PR interval is consistent on the conducted beats.  The QRS complex is narrow.   No ectopic beats are noted.  PR: .20 sec.  QRS:  .08 sec.  QT:  .44 sec.  Interpretation:  2nd degree AV block type II

2.
60 cycle electrical interference

This 60 cycle electrical interference.

3.

The rhythm is regular with a rate of 53/min.  There are flutter waves present.   The QRS complex is narrow.   No ectopic beats are noted.  PR:  ---,  QRS:  .08 sec,  QT:  .44 sec.  Interpretation: Atrial flutter

4.
Demand atrial pacing

The rhythm is irregular.   The rate is 60/min.  Small upright P waves are present.   Most of the P waves are preceded by an atrial pacer spike.  The P wave of the 4th complex does is not preceded by an atrial pacer spike.  The ST segment is down slopping with an inverted T wave.  In lead V1 there is 1 mm ST elevation.  No ectopic beats are noted.  PR:  .20 sec,  QRS:  .10 sec, QT:  .44 sec.  Interpretation:  Demand atrial pacing

5.
Normal sinus rhythm with ventricular escape beats.

The rhythm is irregular.  There are upright P waves present.   The P waves are associated with a QRS complex.  The P wave of the 6th complex is smaller than the other P waves.  It may be from an ectopic source. Ventricular escape beats are seen following the 3rd and 5th complexes.  PR:  .20 sec,  QRS:  .08 sec,  QT:  .40 sec.  Interpretation:  Normal sinus rhythm with ventricular escape beats.
