Sinus arrhythmia with 1st degree block












The rhythm is irregular with a rate of 70/min.   The P waves are upright, uniform, and precede the QRS complexes.  The P waves are split which suggests biatrial enlargement.  The PR interval is prolonged.  The QRS is narrow.  No ectopic beats are noted.   PR:  .24 sec,  QRS:  .08 sec,  QT:  .44 sec.   Interpretation:   Sinus arrhythmia with 1st degree block

 Idioventricular rhythm













The rate is 20/min.  There are no P waves.   The QRS complex is wide which is indicative of a ventricular escape rhythm.  PR:  ---,  QRS:  .18 sec,  QT:  .50 sec.   Interpretation:  Idioventricular rhythm

 Atrial fibrillation with multifocal PVCs













The rhythm is irregular with a heart rate of 100/min.   No P waves are seen, just fibrillation between the QRS complexes.  Multifocal PVCs are seen, the 5th, 9th, and 10th complexes.  The 9th and 10th complexes form a multifocal couplet.   PR:  ---,  QRS:  .08 sec,  QT:  .24 sec. (the rhythm is so irregular it is difficult to measure the QT accurately).   Interpretation:   Atrial fibrillation with multifocal PVCs

Junctional rhythm













The rhythm is regular with a rate of 53/min.   No P waves are seen.   The QRS complex is narrow so this suggests a junctional escape rhythm.   PR:  ---,  QRS:  .10 sec,  QT:  .44 sec.  Interpretation:  Junctional rhythm

 Normal sinus rhythm with a PAC








The rhythm is irregular.   The rate is 90/min.   The P waves are upright and associated with a QRS complex.   The QRS pattern in V1 suggests an incomplete RBBB.   A single PAC is seen, the 4th complex.  PR:  . 16 sec,  QRS:  .10 sec, QT:  .32 sec.   Interpretation:  Normal sinus rhythm with a PAC
