Junctional rhythm












The rhythm is regular with a rate of 60/min.  The P waves are negative and precede the QRS complexes.  The QRS complexes are narrow.   No ectopic beats are noted.   PR:  .20 sec.  QRS:  .08 sec,  QT:  .38 sec.  Interpretation:  Junctional rhythm

Atrial fibrillation with multifocal PVCs













The rhythm is very irregular.  The heart rate is 100/min.   P waves are not present instead there is some fibrillatory waves between the QRS complexes.  There are multifocal PVCs present.  The 2nd and 3rd complex form a multifocal couplet.  The other PVCs are the 4th, 6th, and 9th complexes.  PR:  ---,  QRS:  .12 sec,  QT:  .40 sec.  Interpretation:  Atrial fibrillation with multifocal PVCs

3rd degree heart block  












The rhythm is regular with a rate of 30/min.   The P waves are upright and uniform but not all of them are associated with a QRS complex. The atrial rate is 100/min.  The QRS complexes are wide which points toward an underlying ventricular escape rhythm.  No ectopic beats are noted.  PR:  ---,  QRS:  .16 sec,  QT:  .40 sec.  Interpretation:  3rd degree heart block

 Idioventricular rhythm













The rhythm is irregular with a rate of 30/min.   There are not any P waves.  The QRS complexes are wide.   No ectopic beats are seen.   PR:  ---,  QRS:  .16 sec,  QT:  .44 sec.   Interpretation:  Idioventricular rhythm

Normal sinus rhythm with multifocal PVCs







The rhythm is irregular.  The heart rate is 60/min.  The P waves are upright, uniform, and correspond to a QRS complex.  Multifocal PVCs are present.  PR:  .20 sec,  QRS:  .10 sec, QT:  .44 sec.  Interpretation:  Normal sinus rhythm with multifocal PVCs
