1.

2.

3.

4.

5.

Answers

1.
Normal sinus rhythm with 1st degree block

The rhythm is regular with a rate of 88/min.   The P waves are upright and precede the QRS complexes.   The PR interval is prolonged.  No ectopic beats are noted.  PR:  .28 sec,  QRS:  .08 sec,  QT:  .36 sec.

2.
Sinus tachycardia with trigeminal PVCs

The rhythm is irregular due to the PVCs.   The rate is 110/min.   The P waves are broad and upright and are associated with a QRS complex.  The ectopic PVCs appear to be fused with a sinus P wave.  The R wave of the PVC looks like it begins on what would be a P wave.  If you measure the P-P distance they "march" through the ventricular ectopic beats.  There are trigeminal PVCs.   PR:  .20 sec,  QRS:  .08 sec,  QT:  .36 sec.

3.
Atrial fibrillation with rapid ventricular response

The rhythm is rapid and irregular with a rate of 160/min.  No P waves are present, only fibrillatory activity between each QRS complex.  No ectopic beats are present.  PR:  ---,  QRS:  .08,  QT:  .28 sec.

4.
Atrial flutter with variable ventricular response

The rhythm is irregular with a ventricular rate of  70/min.   Flutter waves are present between the QRS complexes in ratio of 3:1 and 4:1.  No ectopic beats are present.  PR:  ---,  QRS:  .08 sec,  QT:  .40.

5.
Sinus arrhythmia

The rhythm is irregular with a rate of 70/min.  The R-R interval between the first and second complex is .96 sec and the R-R interval between the 4th and 5th complex is .76 sec.   The P waves are associated with a QRS complex.  No ectopic beats are present.  PR:  .16 sec,  QRS:  .08 sec,  QT:  .40 sec.
