Answers

1.
Normal sinus rhythm with 1st degree block and ventricular paced beat every other beat

The rhythm is irregular because of the demand pacing.   P waves are present and are associated with a QRS complex.  The PR interval is prolonged.  The 3rd and 13th complexes are fusion complexes and have a wide QRS complex   There is a ventricular paced beat every other beat.  PR:  .32 sec,  QRS:  .12 sec,  QT:  .32 sec.

2.
Atrial fibrillation with rapid ventricular response

The rhythm is irregular with a rate of 156/min.   No P waves are seen.   There is fibrillatory activity between the QRS complexes.   No ectopic beats are noted.   PR:  ---,  QRS:  .08 sec,  QT:  .28 sec.

3.
Accelerated junctional rhythm

The rhythm is regular.   The ventricular rate is 63/min.  No P waves are seen.  The QRS complexes are narrow.  No ectopic beats are present.  PR:  ---,  QRS:  .06,  QT:  .40 sec.

4.
Normal sinus rhythm with  frequent PACs

The rhythm is irregular.   The rate is 80/min.   The P waves are upright and are associated with a QRS complex.   Some PACs are seen,  2nd, 3rd, 6th, and 8th complexes.  The shape of the ectopic P waves is different from the P waves of sinus origin.  PR:  .12 sec,  QRS:  .08 sec,  QT:  .32 sec.

5.
Ventricular paced

The rhythm is regular with a ventricular rate of 71/min.  There appears to be some fibrillatory activity between the QRS complexes.  No P waves are seen.   No ectopic beats are noted.  A ventricular pacing stimulus precedes each QRS complex.  PR:  ---,  QRS:  .16 sec,  QT:  .36 sec.
