Biventricular paced with a run of VT

The rhythm begins with a biventricular paced beat.  Complex 2 is a PVC but you can see that the pacemaker did not sense the PVC and fired anyway.  This is followed by another biventricular paced beat, complex 3.  Complex 4 looks like a fusion beat.  The pacemaker did not sense the early beat and fires.  Complexes 5 – 14 are a run of VT at a rate of about 150 bpm.  A short compensatory pause follows complex 14 and then biventricular pacing resumes, complexes 15 – 18.
