Answers

1.
Complete heart block

The rhythm is regular.   The ventricular rate is 33/min and the atrial rate is 107.   There are upright P waves and the P waves do not correspond to the QRS complexes.  The QRS complex has an RSR pattern in V1 which is consistent with a RBBB.  There are extra non-conducted P waves.  PR:  --,  QRS:  .12 sec,  QT:  .60 sec.

2.
Accelerated junctional rhythm

The rhythm is regular.   The calculated rate is 65/min.   No P waves are noted in the first two complexes but there are some small retrograde P waves that follow the QRS complexes in the 3rd-7th complexes.  No ectopic beats are present.   PR:  ---,   QRS:  .12 sec  QT:  .44 sec.

3.
Atrial fibrillation

The rhythm is irregular.   The rate is 60/min.   There are no P waves only fibrillatory waves between the complexes.  No ectopic beats are noted.  PR:  ---,   QRS:  .08 sec,  QT:  .44 sec

4.
Bigeminy

The rhythm is regular.   The rate is 100/min.  There are upright P waves present on the sinus beats.    Bigeminal unifocal PVCs are present.  PR:  .12 sec,  QRS:  .12 sec,  QT:  .40 sec

5.
Sinus tachycardia

The rhythm is regular.   The rate is 166.  There are small upright P wave present before each QRS complex.   No ectopic beats are noted.   PR:  .08 sec,  QRS:  .12 sec,  QT:  .28 sec
