If you thought this was torsades de pointe you were close to being right.  Torsades is a form of polymorphic ventricular tachycardia that is characterized by a waveform that begins small and increases in size or amplitude then decreases in size.   The axis of the torsades QRS complexes also change.  The axis of this strip does not really change.  I will put the two strips together for you to compare them.  however, the treatment for both arrhythmias would include defibrillation (if pulseless) and magnesium.  If the patient has a pulse then give antiarrhythmics such as Lidocaine, Procainamide, or Amiodarone.  For torsades also look for underlying causes of the prolonged baseline QT interval such as a low serum magnesium level or other electrolytes and medications known to increase the QT interval.  If the patient is unstable, remember to give unsynchronized shocks rather than synchronized shocks to convert the rhythm.
