NSR with a Couplet of PVCs










The rhythm is irregular due to the PVCs.   The rate is 70/min.  The P waves are upright and precede the QRS complex.  A couplet and a single PVC are seen.   PR:  . 16 sec,  QRS:  .08 sec,  QT:  .32 sec.

Normal Sinus Rhythm with a PJC

The rhythm is irregular because of the ectopic beat.   The P waves have a corresponding QRS complex.  There is an ectopic beat with a negative P wave preceding the 6th QRS complex.  PR:  .16 sec,  QRS:  .08 sec,  QT:  .36 sec.

Normal Sinus Rhythm with a PAC










The rhythm is irregular with a rate of 100/min.   There are small broad P waves before each QRS complex.   An atrial premature complex is seen following the 6th complex.   The P waves of this PAC has a different shape than the P waves of sinus origin.   PR:  .20 sec,  QRS:  .08 sec,  QT:  .36 sec.

Normal Sinus Rhythm with a PJC










The rhythm is irregular.   The rate is 80/min.  The P waves are small and precede the QRS complexes.  There is a PJC that follows closely after the 5th complex.  There is no change in the morphology of the T wave of the preceding complex so this is thought to be a premature junctional rather than an premature atrial complex.  When doubt exists, I have seen then referred to as premature supraventricular complexes.  PR:  .20 sec,  QRS:  .08 sec,  QT:  .28 sec.

Normal Sinus Rhythm with PACs













The rhythm is irregular and the rate is 70/min.   The P waves are positive and precede a QRS complex.  The 3rd, 8th, and 10th complexes are PACs.  The P wave of the PACs has a slightly different morphology than the sinus P waves.  PR:  .12 sec,  QRS:  .08 sec,  QT:  .40 sec.
