 Accelerated junctional rhythm.  

The rhythm is regular with a rate of 94/min.   No P waves are seen.   No ectopic beats are seen. The QRS complex is wide.   PR:  ---,  QRS:  .12 sec,  QT:  .40 sec.  Interpretation:  Accelerated idioventricular rhythm.   The interpretation of accelerated idioventricular rhythm is correct based upon the characteristic of the rhythm.  However, this patient actually had an underlying wide QRS complex on previous strips so the actual interpretation for this strip would be an accelerated junctional rhythm.

Asystole with a functional pacemaker

 There are no P waves are QRS complexes.   There are pacer spikes present but they are not capturing.   Interpretation:  Asystole with a functional pacemaker

Dual paced with biventricular pacing

 The rhythm is regular with a rate of 71/min.   The P waves are not well defined.   There are atrial and biventricular pacer spikes present.  No ectopic beats are noted.  PR:  .12 sec,  QRS:  .16 sec, QT:  .40 sec.  Interpretation:  Dual paced with biventricular pacing

Sinus tachycardia with trigeminal PVCs

 The rhythm is irregular.   The heart rate is 110/min.   There are upright, uniform P waves present.  There are PVCs every third beat, ventricular trigeminy.  A pause follows each PVC.  PR:  .12 sec.  QRS:  .08 sec,  QT:  .32 sec.  Interpretation:  Sinus tachycardia with trigeminal PVCs

Sinus arrhythmia

The rhythm is irregular.  The heart rate is 60/min.   The P waves are upright and associated with a QRS complex.  No ectopic beats are seen.  PR:  .16 sec,  QRS:  .12 sec,  QT:  .44 sec.   Interpretation:  Sinus arrhythmia.
