1.

2.

3.

4.

5.

Answers

1.
Atrial fibrillation with unifocal PVCs

The rhythm is irregular with a rate of 60/min.   P waves are absent but some fibrillatory activity is seen between the QRS complexes.  3 unifocal PVCs are seen, one single and two are paired together.   PR:  ---,  QRS:  .12 sec,  QT:  .36 sec.

2.
Demand ventricular pacing

The rhythm is irregular due to the ectopic beats.   The rate is 90/min.   The P waves are positive and correspond to a QRS complex.   A single PVC is present.  The 2nd and 6th complexes are the patient's own beats.  The patient has an underlying 1st degree block with a PR interval of 26 sec.  The remainder are ventricular paced beats.  The intervals for the paced beats:  PR:  .12 sec,  QRS:  .16 sec,  QT:  .48 sec.

3.
Normal sinus rhythm with triplets of PVCs

The rhythm is irregular due to the PVCs.   The rate is 100/min.   Upright, uniform P waves are associated with a QRS complex.  A pair of triplet PVCs are present.  PR:  16 sec,  QRS:  .06 sec,  QT:  .40 sec

4.
Supraventricular tachycardia

The rhythm is regular and the rate is 150/min.  No P waves or ectopic beats are present.   PR:  ---, QRS:  .06 sec,  QT:  .24 sec.

5.
Polymorphic ventricular tachycardia

The rhythm is irregular with a rate of 220.  No P waves are present.  Multiform ventricular complexes are present.
