Atrial fibrillation with rapid ventricular response











The rhythm is irregular.   The rate varies.  No P waves are clearly identified.  No ectopic beats are noted.  The paroxysmal pattern helps to identify the rhythm.  During the periods of slower conduction the fibrillatory waves are clearly seen.  PR:  ---,  QRS:  .08 sec, QT:  .28 sec

Junctional rhythm
 The rhythm is regular with a rate of  51/min.  In the first three complexes there are no P waves but in the last three complexes some small retrograde P waves emerge.  No ectopic beats are present.  PR:  ---,  QRS:  . 08 sec,  QT:  .38 sec.

Sinus tachycardia with a PAC and a couplet

The rhythm is irregular due to the various ectopic beats.   The rate is 130/min.  Upright P waves are present and are associated with QRS complexes.  The PAC is the 2nd complex.   The P wave is hidden within the T wave of the preceding complex.  Towards the end of the strip is a couplet that is followed by an escape beat.  PR;  .16 sec,  QRS:  .08 sec,  QT:  .32 sec

Trigeminal PVCs
The rhythm is irregular due to the PVCs.   The rate is 80/min.  The P waves are upright and uniform.   There are three unifocal PVCs that occur every third beat.   PR:  .12 sec,  QRS:  .08 sec,  QT:  .56 sec.

Complete heart block with unifocal PVCs






The unifocal PVCs occur every 5th beat (pentageminy).  There is no relationship between the P waves and the QRS complexes.  Some P waves can be seen in the terminal part of the S wave of the PVCs as well.  PR:  ---,  QRS:  .12 sec,  QT:  .44 sec.
