4.

5.

Answers

1.
2nd degree heart block type I

The rhythm is irregular with a rate of 60/min.   There are P waves present but the P wave after the 3rd complex is not conducted.  The PR interval increases over successive beats.  No ectopic beats are noted.  PR: ---,  QRS:  .08 sec,  QT:  .44 sec.

2.

There is complete AV dissociation.   The atrial rhythm is regular with a rate of 60/min and the ventricular rhythm is also regular with a rate of 40/min.   The P waves are dissociated from the QRS complexes.  No ectopic beats are noted.  PR:  ---,  QRS:  .16 sec,  QT:  .44 sec.  If the patient is unstable, consider transcutaneous pacing or begins dopamine or an epinephrine drip as an alternative to pacing.

3.
Complete heart block

The is complete AV dissociation.  There is some irregularity in the P-P interval.  The atrial rate is 70/min.   The ventricular rate is 40/min.  No ectopic beats are noted.  PR:  ---,  QRS:  .12 sec,  QT:  .48 sec.

4.
1st degree block

The rhythm is regular with a rate of 68/min.  The P waves are associated with a QRS complex and are upright.  The PR interval is prolonged at 0.28 seconds.  No ectopic beats are noted.  PR:  .28 sec, QRS:  .08 sec,  QT:  .40 sec.

5.
2nd degree heart block type II

The rhythm is regular.   The rate is  34/min.  The P waves are upright but not all of them are associated with a QRS complex.  There is a nonconducted P wave after each QRS complex. The PR interval is consistent on the conducted beats.  No ectopic beats are noted.  PR:  .28 sec,  QRS; .08 sec.  QT:  .44 sec.
