1.

2.

3.

4.

5.

Answers

1.
2nd degree AV block type I

The rhythm is irregular with a rate of 50/min.   There are upright P waves present.  There is a dropped P wave after the 2nd complex.  The PR interval increases over successive beats.  No ectopic beats are noted.  PR:  ---,  QRS:  .08 sec,  QT:  .48 sec.

2.
Complete heart block

The rhythm is regular with an atrial rate of 70/min and a ventricular rate of 30/min.   The P waves are uniform and upright but are not associated with the QRS complexes.  No ectopic beats are present.  PR:  ---,  QRS:  .16 sec,  QT:  .48 sec.

3.
2nd degree AV block type II

The rhythm is irregular with a rate of 60/min.   There are P waves that have a corresponding QRS complex but there are also some nonconducted P waves present.  The PR interval is consistent on the conducted beats.  PR:  .24 sec,  QRS:  .14 sec,  QT:  ---

4.
Wide complex tachycardia
You can see that the patient's underlying rhythm (right side of the strip)  has a wide QRS complex.   So when this patient had some runs of SVT it would look like ventricular tachycardia on the monitor.

5.
Normal sinus rhythm with a PAC

The rhythm is irregular with a rate of 60/min.  The P waves are upright and are associated with a QRS complex.  There is a PAC that follows the 3rd complex.  The shape of the P waves differs from the sinus P waves.  PR:  .16 sec,  QRS:  .08 sec,  QT:  .36 sec.
