Junctional rhythm










The rhythm is regular with a rate of 39/min.   No P waves are seen.   There are no ectopic beats.  PR:  ---,  QRS:  .08 sec,  QT:  .56 sec.


2.
Normal sinus rhythm with PACs










The rhythm is irregular with frequent ectopic beats.  The P waves are upright and associated with a QRS complex.  There are some ectopic beats, the 4th, 7th, and 10th beats.  The 4th and 7th ectopic beats have a wider QRS complex and may represent some aberrantly conducted PACs.  A small upright P waves precedes these two complexes.  The last ectopic beat has a P wave that is hidden in the T wave of the precedeing complex and it has a more narrow QRS complex.  PR:  .16 sec,  QRS:  .08 sec,  QT:  .44 sec.

NSR with 1st degree block and multifocal PVCs










The rhythm is irregular with a heart rate of 70/min.  The P waves are positive and have a corresponding QRS complex.   Two multifocal PVCs are seen.  PR:  .22 sec,  QRS:  .08 sec,  QT: .36 sec.

NSR with trigeminy










The rhythm is irregular with a ventricular rate of 100/min.   Upright sinus P waves are seen.  A QRS complex follows each P wave.   Unifocal PVCs are seen every third beat.   PR:  .20 sec,  QRS:  .08 sec,  QT:  .40 sec

NSR with bigeminy












The rhythm is irregular.   The heart rate is 70/min.   The P waves are upright and are associated with a QRS complex.   Unifocal PVCs occur in a bigeminal pattern.  A compensatory pause follows each PVC.   PR:  .12 sec,  QRS: .10 sec,  QT:  .44 sec.
