1. d. Atrial fibrillation with run of VT. The underlying rhythm is irregular. No P waves are seen, just some fibrillation between the QRS complexes.
2. b. Agonal rhythm.  The QRS complexes are wide an bizzare looking. The heart rate is about 18 bpm

4. a. Normal sinus rhythm. The actual heart rate is about 68 bpm.
