Agonal rhythm
 No P waves are present.  The QRS is wide and the rate is very slow in this bradyasystolic rhythm.

Atrial fibrillation with slow ventricular response.

The rhythm is irregular with a ventricular rate of 160/min.   No P waves are present but some some fibrillation is seen between the QRS complexes as the rate slows down.   No ectopic beats are seen.  PR:  ---,  QRS:  .08 sec,  QT:  .28 sec.

3rd degree heart block
 The rhythm is irregular.   The ventricular rate is 33/min and the atrial rate is 115/min.   The P waves are not associated with the QRS complexes.  The QRS complexes are wide suggesting a ventricular escape rhythm.   A single PVC is seen, the 2nd complex.  PR:  ---,  QRS:  .12 sec,  QT:  .56 sec.

Junctional tachycardia

The rhythm is regular with a heart rate of 150/min.   The P waves are negative and precede each QRS complex.  The QRS complex is narrow.   No ectopic beats are seen.   PR:  .12 sec,  QRS:  .08 sec,  QT:  .36 sec.

NSR with multifocal PVCs














The rhythm is irregular with a rate of 70/min.  The P waves are upright and precede each QRS complex.  Multifocal PVCs are present.   A compensatory pause follows each QRS complex.  An atrial escape beat follows the 2nd PVC.   PR:  .20 sec,  QRS:  .10 sec,  QT:  .42 sec.
