1. 2nd degree heart block type II with a couplet of PVCs. With 2nd degree heart block the PR interval on the conducted beats is constant. Then there are extra nonconducted P waves. Two consecutive PVCs is described as a couplet of PVCs

2. Sinus bradycardia. There is one P wave for each QRS complex and the PR interval is less than .20sec. The rate is 39 and the rhythm is regular.

3. Ventricular fibrillation. The QRS complexes are disorganized and change in amplitude. Ventricular fibrillation may be described as coarse, fine, or very fine. Defibrillation is the treatment of choice for this rhythm.

4. Agonal rhythm. A wide bizzare, slow QRS indicates that this rhythm is ventricular in origin. Usually associated with a dying heart.

5. Sinus tachycardia. The rate is over 100. The rhythm is regular. There is one P wave for each QRS complex.
