Sinus Tachycardia with Frequent Unifocal PVCs

Sinus Tachycardia with Frequent Unifocal PVCs.  The underlying rhythm is sinus in origin.  The ectopic beats are wide and lack a P wave so this makes them ventricular in origin.  When the PVCs all have a similar morphology they are said to be unifocal PVCs

Normal Sinus Rhythm with trigeminy of PVCs








Again the underlying rhythm is sinus in origin.  Ventricular trigeminy occurs when there is a PVC every 3rd beat.

Normal Sinus Rhythm with Premature Junctional Contractions








In looking at the rhythm, one can see that it is irregular.  If you were to pick out the complexes that appear irregular, you would notice that they are narrow and that they have an inverted P wave, the 3rd, 7th, and 11th complex for example.  They fit the characteristics of premature junctional beats.  As in junctional rhythms, the P waves may be inverted, be absent, or follow the QRS complex.

Normal Sinus Rhythm with a Premature Atrial Contraction








Again there is an underlying sinus rhythm that appears irregular. If you examine the P waves before the 6th, QRS complex you will notice that it appears different from the other P waves.  It appears taller than the rest. This sometimes happens when the P wave falls on the T wave of the preceding complex.

Sinus bradycardia with a PJC, PAC, and a PVC











Sinus bradycardia with a PJC, PAC, and a PVC.  The 2nd complex is a PJC and it is identified by the inverted P wave that precedes the QRS complex.   The PAC is the 4th complex and it has a P wave that has a different shape than the other sinus P waves.  The PVC is the 6th complex.
