Junctional rhythm










The rhythm is regular.   The heart rate is 40/min.   The P waves are not seen.  No ectopic beats are noted.  The QRS complex is narrow.  PR:  ---,  QRS:  .08 sec,  QT:  .56 sec


2.
NSR with trigeminal PACs










The rhythm is irregular with a heart rate of 100/min.   The P waves are upright and have a corresponding QRS complex.  Premature atrial complexes are seen every 3rd beat.  The P wave of the PACs is fused with the T wave of the preceding complex.  PR:  .18 sec,  QRS:  .08 sec,  QT:  .38 sec.

NSR with 1st degree AV block and multiple ectopic beats and atrial escape beats










The rhythm is irregular.   The heart rate is 80/min.   The P waves are upright and are followed by a QRS complex.  Multifocal PVCs are seen.  The 1st and 2nd ectopic beats have a small P wave present and likely represent PACs with aberrant conduction.  The 3rd PVC is followed by a long pause and then an atrial escape beat, the 9th complex.  Other atrial escape beats are also present.   The 1st and 5th complexes are preceded by a pause as well and have a similar P waves and morphology as the 9th complex.  PR:  .24 sec,  QRS:  .10 sec,  QT:  .46 sec.

NSR with unifocal PVCs










The rhythm is irregular due to the multiple PVCs.   The heart rate is 80/min.  The P waves are uniform and upright.   Unifocal PVCs are seen every 3rd beat.  PR:  .16 sec,  QRS:  .08 sec,  QT:  .44 sec.

Ventricular paced rhythm










The rhythm is regular with a ventricular rate of 83/min.  No P waves are seen.  Ventricular pacing stimuli are seen before each QRS complex.  The QRS complex and the T wave have opposite polarity which is typical for a ventricular paced rhythm.  PR:  ---,  QRS;  .16 sec,  QT:  .44 sec.
