Sinus tachycardia with wandering baseline













The rhythm is regular with a rate of 115/min.  Upright P waves are seen before each QRS complex.   No ectopic beats are present.   There is wandering baseline.   P:  .12 sec,  QRS:  .06 sec,  QT:  .32 sec.

2nd degree AV block type II













The rhythm is regular.   The atrial rate is 80/min and the ventricular rate is 40/min.  Up right P waves are present but some P waves are not associated with a QRS complex.   The PR on the conducted P waves is consistent.   PR:  .16 sec,  QRS:  .10,  QT:  .40 sec.

Atrial fibrillation with rapid ventricular response













The rhythm is irregular and the rate is 160/min.   No consistent P waves are present but some fibrillatory activity can be seen between the QRS complexes.  PR:  ---,  QRS:  .06 sec,  QT:  .24 sec.

Atrial fibrillation with slow ventricular response













The rhythm is irregular with a rate of 50/min.   No P waves are present.  Between the QRS complexes there are some fibrillatory waves.   No ectopy is noted.  PR:  ---,  QRS:  .08,  QT:  .40 sec.

Normal sinus rhythm with ventricular trigeminy














The rhythm is irregular due to the PVCs.   The rate is 90/min.   There are upright P waves present which are accompanied by a QRS complex.  Unifocal PVC occur every third beat.   PR:  .18 sec,  QRS:  .10 sec,  QT:  .40 sec.
