Atrial fibrillation with a PVC








The rhythm is irregular with a ventricular rate of 78/min.   There are no P waves but there is some fibrillatory activity between the QRS complexes.  A single PVC is seen.   PR:  ---,  QRS,  .16 sec,  QT:  .48 sec,

Atrial fibrillation with slow ventricular response
 My first inclination was to call this a junctional rhythm but because it is so irregular and that there appears to be fine fibrillatory waves in between the complexes, I have to identify it as an atrial fibrillation.   And that was exactly the case.   The patient had a history of afib and came into the hospital with unstable bradycardia.  He was taking Labetalol to control his afib.   The MD treated him with Dopamine for his heart rate and BP as well as Glucagon for the beta blocker toxicity.

NSR with unifocal PVCs










The rhythm is irregular due to the PVCs.   The rate is 90/min.   The P waves are upright and preceded the QRS complexes.   Two unifocal PVCs are seen.   PR:  .12 sec,  QRS:  .08 sec,  QT:  .40 sec.

Supraventricular tachycardia








The rhythm is regular with a rate of 150/min.   No P waves are seen.  There are no ectopic beats.  The ST segment is up slopping.  PR:  ---,  QRS:  .10 sec,  QT:  .36 sec.


5.
Ventricular paced
The rhythm is regular with a ventricular rate of 79/min.   P waves are upright and precede the QRS complex.  No ectopic beats are seen.  A ventricular pacing stimulus precedes each QRS complex.  PR:  .12 sec,  QRS:  .16 sec,  QT:  .48 sec.
