1.
Atrial fibrillation with couplets of PVCs










The rhythm is irregular with many PVCs.   The rate is 100/min.  The top lead is lead II and the bottom lead is V1.   No P waves are identified.   There is fibrillatory activity between the QRS complexes.   Three couplets are seen.  PR:  ---,  QRS:  .12 sec,  QT:  .32 sec.

Ventricular paced










The rhythm is regular with a rate of 75/min.   The P waves are absent.   There appears to be some fibrillatory activity between the QRS complexes.   A ventricular pacing stimulus precedes each QRS complex.  PR:  ---,  QRs:  .16 sec,  QT:  .44 sec.

NSR with PACs










The rhythm is irregular due to the atrial ectopy.   The rate is 60/min.   P waves are seen and precede each QRS complex.   There are two PACs present,  the 5th and 8th complexes.   The P waves of the PACs has a different shape than the sinus P waves.   PR:  .12 sec,  QRS:  .08 sec,  QT:  .44 sec.

Sinus bradycardia with a sinus pause










The rhythm is irregular due to the pause.   The rate is 50/min.   The P waves are upright and have a corresponding QRS complex.   No ectopic beats are seen.   The ST segment is up slopping in the top lead (II) and 0.5mm ST depression with T wave inversion is seen in the bottom lead (V1).  A  1.44 sec pause is seen after the 6th complex.  PR;  .20 sec,  QRS:  .08 sec,  QT:  .52 sec.


5.
Sinus bradycardia with ST depression and inverted T waves









The rhythm is slightly irregular with a rate of 52/min.   The P waves are uniform and upright.   No ectopic beats are noted.   the T waves are inverted in lead II.  PR:  .16 sec,  QRS:  .12 sec,  QT:  .44 sec.
