Answers

1.
NSR with bigeminy

The rhythm is irregular due to the PVCs.  The rate is 90/min.   The P waves are wide, upright and precede a QRS complex.  There are unifocal PVCs that occur every other beat.   A compensatory pause follows the PVC.  PR;  .16 sec.  QRS:  .12 sec,  QT:  .36 sec.

2.
Sinus tachycardia

The rhythm is regular with a rate of 102.   The P waves are wide and tall suggesting biatrial enlargement.  No ectopic beats are noted.  PR:  . 16 sec,  QRS:  .08 sec,  QT:  .36 sec.

3.
NSR with a PAC

The rhythm is irregular with a PAC.  The rate is 90/min. The P waves are upright and precede the QRS complexes.  The 8th complex is a premature atrial complex.  The morphology of the P waves differs from the sinus P waves.  PR:  .20 sec,  QRS:  .08 sec,  QT:  . 36 sec.

4.
Atrial fibrillation with slow ventricular response

The rhythm is irregular.  The rate is 36/min.  (This is a 10 second strip.  Multiply the number of QRS complexes in the strip by 6).   No P waves are noted.   The QRS complex is narrow.   PR:  ---,  QRS:  .08 sec,  QT:  .44 sec.   The irregularity of the rhythm and the absence of P waves suggests atrial fibrillation instead of a junctional rhythm which tends to be more regular.

5.
AV paced

The rhythm is regular with a rate of 72/min.  The paves and the QRS complexes are preceded by pacer spikes.  No ectopic beats are noted.  PR;  .10 sec,  QRS:  16 sec,  QT:  .44 sec.
